The Mobile Team Primary Care is already established in Skåne and is experienced very positively. In September 2024, we launched and further developed this concept in Lund as well, with the catchment area of Middle Skåne. The healthcare providers we collaborate with include Sus Lund, CSK, Läkarbil Skåne, ambulance services, private and public health centers, as well as other healthcare providers in the area, such as the care and elderly services administration in the municipalities.

The Mobile Team Primary Care is a cohesive form of care where Primary Care is the hub in collaboration with other healthcare providers. The team offers safe, clear, and accessible healthcare at home for frail and multi-morbid patients. The team can assess, plan, prescribe, and perform point interventions. The care is specialized based on each individual patient's needs, and the goal is to offer patients qualified care both medically and psychosocially.

The team operates from premises in Lund and consists of doctors, nurses, social workers, and a medical secretary.

RESPONSIBILITIES

As a nurse, you provide healthcare in the patient's home, as our goal is to both assess and treat the patient at home. You lead and actively participate in planning the patient's daily care together with other professions and collaborate with other healthcare providers. You are also expected to actively participate in the organization's development and change work.

QUALIFICATIONS

We are looking for you who are a licensed nurse with the eligibility to work within Swedish healthcare. Knowledge of Swedish at least corresponding to level C1 according to the Council of Europe's scale is a requirement. You have previous experience working as a nurse, and for this position, it is a requirement that you have a B-driver's license, as the care is mainly provided in the patient's home. We consider it meritorious if you have experience in any of the following: internal medicine, infectious disease care, inpatient care, municipal health and medical care, primary care, or other experience, as we desire a breadth of competence in the team.

Employment type: Permanent.
Duration: Permanent.
Working hours: Daytime. Working hours are 08:00-17:00, Monday - Sunday. As the schedule currently stands, nurses work every tenth weekend and one to two red days per year, where nurses have proportionate working time measures.

The Department of Close Care and Health has existed since January 1, 2025. The department is responsible for grant-funded healthcare activities within close care. This includes among other things mobile teams and specialized palliative care, reception activities within close care, emergency care within close care, 1177 by phone, and some region-wide common functions. Additionally, the department is responsible for the development and follow-up of the transition to close care in Region Skåne. The number of employees is estimated to be around 1,000.
