Target Group
The target group consists of boys and girls aged 16–20 with psychosocial issues, neuropsychiatric diagnoses (NPD), or those needing support in the transition from residential care (HVB) or foster homes to a more independent life. The young people should primarily be able to manage their daily lives with support, but not require the more intensive treatment efforts offered by residential care.
About the Role
As a Supported Housing Worker, you are the contact person, provide support in daily tasks, document interventions, and collaborate with authorities, healthcare, and other actors. We are looking for you with post-secondary education in social work, treatment pedagogy, or similar, as well as experience working with young people. Knowledge of CBT, clarifying pedagogy, MI (Motivational Interviewing), or low-arousal approach is advantageous.
As a person, you are secure, empathetic, flexible, and solution-focused. Good knowledge of documentation is also required, including medical records, creating implementation plans, and writing monthly reports. The position involves scheduled working hours during the day, evening, and weekends, including on-call sleeping shifts. A valid driver's license (Category B) is required. Interviews are conducted on an ongoing basis.
About the Facility
We receive clients on assignment from social services across the country. We offer a secure and home-like housing environment with individual interventions based on implementation plans and individual goals. This aims to increase the conditions for providing the young person with opportunities, strategies, and resources for the future after the placement ends.
Fogdaröd is beautifully located in a park with proximity to nature. In the adjacent forest, there are beautiful waterways and exercise trails that contribute to an active leisure time. We also have outdoor and indoor gyms on the premises that the young people have the opportunity to use.
Your Responsibilities
In the role of Supported Housing Worker, you work with overarching interventions in the young person's daily life, such as:
- Together with your colleagues, be responsible for good structure and that the housing's routines are pursued and followed to create a secure and predictable daily life for our young people.
- Support and work with interventions related to ADL (Activities of Daily Living).
- Participate in pedagogical meals with the young people.
- Collaborate with schools, guardians, and other care institutions as needed.
