The position is full-time, daytime Monday–Friday. It is distributed approximately 80% within the Silvia/Dementia assignment and approximately 20% clinical work in our operations within municipal primary care.

WORK ASSIGNMENTS

As a Silvia Nurse/Dementia Nurse, you work consultatively, supportively, and developmentally within the entire area of operations. You are an important knowledge carrier and collaboration partner for both patients and relatives as well as for staff, health centers, and other actors.

You will, among other things:

  • Coordinate and follow the flow regarding people with cognitive illness
  • Participate in collaboration with health centers and other professions
  • Perform memory tests and conduct home visits
  • Provide support to patients and relatives through counseling, conversations, and educational efforts
  • Work actively with the BPSD register and educate both administrators and other staff
  • Supervise and educate licensed personnel
  • Participate in networks and collaboration forums at local, regional, and national levels
  • Contribute to development and quality assurance within the municipality's care for cognitive illness
  • Participate in improvement work, process leadership, and implementation of new working methods
  • Keep educational material up to date and spread new knowledge within the field
  • Contribute to the work with action plans and long-term development of the municipality's care within the field of cognitive illness

The role also includes clinical work within municipal primary care where you contribute your competence close to the operations and the patients.

QUALIFICATIONS

We are looking for you who are a licensed nurse with further education in dementia/cognitive illness or other relevant specialist training. You have a great interest in person-centered care and want to help develop dementia care in the municipality.

The following are required for the position:

  • Swedish nursing license
  • Trained as a Silvia Nurse or specialized in care for cognitive illness/dementia
  • Good documentation skills and good computer skills
  • Good Swedish in speech and writing
  • Driver's license category B

The following is meritorious:

  • Are a certified trainer in the BPSD register
  • Have experience of municipal health and social care
  • Have experience of supervision, education, and improvement work
  • Have experience of collaboration between different operations and professions

You are structured, solution-focused, and have the ability to drive processes from idea to implementation and follow-up. The role requires that you can coordinate, inspire, and motivate others while working close to the operations.
