Are you a diabetes nurse? Do you want to run and develop our diabetes clinic together with fellow diabetes nurses and collaborate closely with our doctors to care for our diabetes patients? Then this position is for you!
Health Centre Centrum is a unit in development with approximately 13,000 registered patients, making us one of the largest health centres in Scania. Here, a cross-professional team works together, consisting of specialist doctors, resident doctors, nurses, nursing assistants, social workers, psychiatric nurses, occupational therapists, physiotherapists, and dietitians.
We are in constant development where patient needs and medical quality are central.

RESPONSIBILITIES
As a diabetes nurse here, you will meet various types of diabetes patients and work with, for example, adult patients with diabetes, newly diagnosed diabetics, and patients with prediabetes. The work includes planned follow-ups of our diabetes patients as well as registrations in the National Diabetes Register (NDR).
The position also involves active engagement in the development of our diabetes clinic with high medical quality and professionalism, and you will be part of a team consisting of two colleagues and doctors.
The role includes continuously keeping up-to-date with knowledge in the diabetes field and having close collaboration with doctors, dietitians, and other staff at the health centre to offer patients evidence-based diabetes care.
To support your professional development, we offer regular and relevant further education.
In addition to your own clinic work with diabetes patients, the role also includes other duties at the health centre, such as telephone advice and triage.
QUALIFICATIONS
We welcome you who are a licensed nurse, or district nurse, with the right to practice in Swedish healthcare. Additionally, you have advanced education as a diabetes nurse. Proficiency in Swedish corresponding to at least level C1 according to the Council of Europe's scale is a requirement. Furthermore, it is meritorious if you have work experience from primary care and/or experience working in the medical record system PMO.
