SUS Hospital at Home - Malmö is run by the Emergency Care and Internal Medicine department together with the Infectious Diseases department. The form of care means that patients who wish it and who are in need of acute specialist care can be offered care in their own home.

Inspiration for the working method came from other countries where the concept "Hospital at home" is used successfully. Studies show several positive effects for the patient cared for in their own home, such as the patient eating and sleeping better, being more active, and the risk of acute confusion or falls decreasing. Experiences so far show very high patient satisfaction and patient participation.

Job Description

We are now looking for you, a nurse, who wants to contribute to further developing acute hospital care at home.

Instead of the patient being admitted to a care unit at the hospital, we as healthcare personnel will go to the patient's home to provide the care that is currently performed on inpatient wards. Patients can be followed remotely via digital tools and can always quickly contact us at Hospital at Home and be admitted to the hospital if the condition requires it.

We have and continue to strive for broad competence in the team and value this highly as our patients can come from all different units within the hospital, even though most are treated for infections requiring antibiotics. We want to constantly develop and work actively with which patients we can care for and how we can include and take care of as many patients as possible in the best way.

The position primarily involves independent work with patients in their homes where we perform nursing treatments and assessments as well as medical treatments and assessments. We can handle much of the medical measures that can be performed on a regular care unit inside the hospital. Our patients come directly from the emergency department, from care units, and directly from primary care. It may also involve treatments at the hospital for our patients registered in Hospital at Home. Some digital work is also included, such as video rounds with patients and coordination of patients inside the hospital.

You are part of a competent team with nurses, doctors, and rehabilitation staff, but the position involves independent work at the patients' homes. In addition to nursing assessments and measures, you will also support the patient with how digital tools work.

Working hours will be scheduled for day and evening shifts, both weekdays and weekends, without night duty.

Qualifications

We are looking for you who are a registered nurse with permission to work within Swedish healthcare. Language skills in Swedish at least corresponding to level C1 according to the Council of Europe scale are a requirement. You have previous experience working as a nurse, and for this position, it is a requirement that you have a B driving license.

It is meritorious if you have experience in any of the following: internal medicine, infectious disease care, inpatient care, municipal health and medical care. Since one of your main work tools will be digital aids, we see it as positive if this is something you are used to working with and if you can explain technical aids in a pedagogical way.
