The Social and Care Administration in Katrineholm Municipality works with support, care, and authority functions for people at different stages of life. Our mandate is broad and spans several operations, from elderly care and home healthcare to individual and family care as well as guardianship services.

We are now looking for case workers for the authority group

The target group for the position is primarily within elderly care. Your future workplace is within the Authority for Elderly and Functional Support, a unit within the Adult Division of Individual and Family Care. The Authority Group consists of nine case workers within SoL elderly care, five case workers within LSS and social psychiatry, one senior case worker/coordinator, and one case worker for housing adaptations. The unit also includes support for relatives, leisure, and culture within elderly care, as well as personal ombudsmen.

The unit is in an exciting phase of development due to the new Social Services Act, where the focus lies more heavily on preventive work. We have a great interest in developing digital opportunities in the operations. Together, we take responsibility for developing the operations based on set goals, working systematically with quality issues, contributing to a good work climate, and always having a clear user perspective – with the user at the center.

Job Responsibilities

As a case worker according to the Social Services Act (SoL), you are responsible for receiving applications, investigating and assessing needs, making decisions on interventions, and following up on these according to current legislation. In your work, you have the individual's needs at the center and contribute to ensuring that the service provided is legally secure, efficient, and of good quality.

You conduct investigations by obtaining and analyzing relevant information and make decisions according to delegation. The work also involves independently planning your workday and advancing your cases, while maintaining close cooperation with colleagues in daily work.

The role involves extensive collaboration, both internally and externally. You have ongoing contact with other operations within the municipality as well as external partners, such as the region's health and medical care. An important part of the work is collaboration with hospitals within Region Sörmland in connection with patient discharge, where communication takes place via the operation system Cosmic Link.

We work with needs assessment according to the IBIC model, which visualizes the user's needs, activities, and participation. Documentation in individual cases takes place in the operation system Treserva. In your role, you represent the municipality and meet users and relatives in a professional and trust-building manner.

Qualifications

We are looking for you who have a degree in social work or social care, as well as experience in authority functions and case handling according to current legislation. Experience working with IBIC is meritorious. You are computer literate, feel confident documenting in operation systems, and express yourself well in Swedish, both orally and in writing. A B-driver's license for manual transmission is a requirement.

To thrive in the role, you are self-reliant and take responsibility for driving your work forward in a structured and efficient manner. You are flexible and adapt easily when prerequisites change, seeing opportunities and contributing with a non-hierarchical approach. The job places high demands on collaboration skills as you cooperate with various providers and other adjacent operational areas. You are receptive in your communication, build good relationships, and contribute to a good collaborative climate.

The current position is appointed subject to the decisions that are required. According to the Social Services Act, extracts from the police criminal record must be presented by persons before they are employed to work in homes for care or housing (HVB homes), supported housing, or protected housing.
