The elderly care consists of three geographical areas: Svalöv, Teckomatorp, and Kågeröd. Within each area, there are specialized housing facilities and home care services. Here, nurses, occupational therapists, physiotherapists, healthcare assistants (HSL), and rehabilitation assistants work in teams based out of the specialized housing facilities.

Svalöv is part of the Medelpunkten municipal association, whose mission is to meet the member municipalities' needs for assistive devices for which the municipalities are responsible. As an occupational therapist, you can benefit from Medelpunkten's knowledge and experience regarding assistive devices and prescriptions.

Together with the Svalöv Health Center, the home healthcare service has a dedicated team. This means that patients living in one of our specialized elderly housing facilities, LSS group homes, or receiving nurse-led home healthcare are offered, among other things, good access to doctors. The collaboration is close, and we continuously work to develop our home healthcare team to provide close and high-quality care to our patients.

Job Responsibilities

You work with home healthcare for people living in specialized elderly housing, LSS group homes, social psychiatry housing, or ordinary housing. You have a central function in collaboration with and around the patient and ensure good quality in nursing care. Documentation takes place in our operational system, Lifecare HSL.

As a licensed occupational therapist, you work independently and/or together with other professional categories in the team to strengthen the individual's independence and increase/maintain functions and participation in daily life. Your work includes, among other things, ADL assessments, functional assessments, home adaptations, prescription of assistive devices, as well as rehabilitation and habilitation together with the patient. You assess, plan, follow up, and evaluate the health care that patients need.

You also provide support and advice to nursing staff, delegate health care tasks, and lead initiatives within your area of expertise.

The workday consists of planned interventions and home visits; sometimes unplanned events occur that require you to be able to reschedule your work. The work is independent and flexible, and you contribute to developing a care and service that we can all be proud of.

Qualifications

We welcome all licensed occupational therapists to apply. It is meritorious if you have experience from home healthcare, are confident in your professional role, and have the ability to collaborate, plan independently, and prioritize in your work.

You must have good computer skills and a good command of the Swedish language in both speech and writing.

Great importance is attached to personal suitability and good references from previous workplaces.

A Category B driver's license is required.
