Vårdcentralen Tollarp, branch Degeberga, are clinics located in the countryside, next to forests and open fields, and we have approximately 8,300 registered patients. Both locations have a strong village feel and a high level of trust in the health center. Here we offer care for both somatic and mental health issues and work on developing new approaches based on team-based care. We are connected to five nursing homes and provide home healthcare in two areas.

The medical team consists of general specialists and trainee doctors. In addition to the doctor's office, we have physiotherapy, a counselor, a dietitian, a psychologist, and an occupational therapist, nursing assistants, as well as nurse-led specialist clinics for asthma/COPD, diabetes, elderly care, child health services (CHS), and health talks. Experienced nursing assistants work in our sample collection unit, and medical secretaries, receptionists, and administrators provide important support in daily operations.

We are constantly developing, with a focus on further enhancing teamwork. We strive to offer patients cross-professional assessments based on their needs from professional roles such as physiotherapist, psychologist, counselor, and occupational therapist. We actively work to fully utilize everyone's competencies. Through this, we can together provide the best care and use our resources optimally when the patient sees the right person directly.

WORK RESPONSIBILITIES

We are now welcoming a nursing colleague with a focus on asthma/COPD who also has a genuine interest in primary care.

As a nurse, you will work with, among other things, your own clinic for asthma/COPD, telephone advice, patient triage, as well as digital e-services and chat. Based on your previous experience and interests, other tasks may also be included in the position. We offer you stimulating work with varied tasks and a good sense of community. As a nurse, you work independently and in close collaboration with your colleagues.

QUALIFICATIONS

We welcome you who are a licensed nurse, or district nurse, with the right to practice in Swedish healthcare. Further education in asthma/COPD is strongly meritorious for the position. Language skills in Swedish equivalent to at least level C1 according to the Council of Europe scale are required. A valid B driving license is required as travel occurs in the role.

It is meritorious if you have experience from working in primary care and/or experience working in the medical record system PMO.

You enjoy working independently while also finding it easy to collaborate with others. Furthermore, you are receptive and driven to provide good and quality care.
