The Rehab Hotel is located in the premises of the Old Karolinska University Hospital (GKS) in Solna and operates ASiH (Advanced Home Healthcare) and SPSV (Specialized Inpatient Care) on behalf of Region Stockholm.

We work in a multidisciplinary team consisting of nurses, doctors, nursing assistants, physiotherapists, occupational therapists, dietitians, and social workers. Upon admission, we have overall responsibility for the patient's situation, combining general care with specialist competence in healthcare and rehabilitation. Patient categories within our operations include: heart disease, COPD, asthma, cancer, infection, hematology, and nutritional issues.

Our patients reside in central Stockholm, inside the ring road, Hammarby Sjöstad, Lidingö, and Solna.

We are looking for a social worker who wants to be involved in patient care with a holistic view and understanding of the situation the patient and their relatives are in. You enjoy collaborating in a team to achieve the best possible care for the patient, where the patient's well-being is always the focus. You are flexible and accustomed to solving problems, both independently and in dialogue with other professions.

Qualifications: Good knowledge of the Swedish language, both spoken and written. Meritorious: Experience from ASiH, palliative care, home healthcare, and geriatrics.

Job Responsibilities

The work involves both visits to patients' homes and our SPSV ward. The responsibilities include psychosocial support, counseling for patients and relatives, as well as counseling for mild depression and anxiety. It also includes standard practical social support measures, such as contact with social secretaries, home care services, etc., as well as collaboration with other healthcare providers.

The responsibilities also include supporting the collegial team through individual conversations in special situations and group discussions, such as ethical dialogues.
