We are a stable and well-functioning unit with 10,600 registered inhabitants, operating in an area with a unique combination of multicultural city life and rural Scania. The doctor's office includes six specialists in general practice and seven trainee doctors (ST, AT, and BT). We are a multifaceted operation with clinics for assistant nurses, district nurses, physiotherapists, counselors, dietitians, and occupational therapists. Here you will find specialized nurse-led clinics for asthma/COPD, diabetes, heart failure, cognitive impairment, and targeted health talks. There is also a well-developed full-time rehabilitation coordinator function, an elderly care clinic, a lifestyle clinic, and a mobile care team. Initial care is handled smoothly in telephone advice and primary care triage by nurses, supported by doctors, physiotherapists, and assistant nurses. Interpreters and service hosts are available in the waiting room.

RESPONSIBILITIES

In the role of nurse/district nurse, you will work with telephone advice, handling of acute patients, care plan adjustments, as well as medical administration and clinic work. You will also work with triage. Naturally, you will receive internal training based on your existing competence and areas of interest, all to meet the patients' needs in the best possible way.

Here, you will become part of a lovely team of nurses and district nurses, where we are about fifteen colleagues who collaborate and exchange experiences. We strive for a team-oriented approach and continue our improvement work to meet our patients in the best way - do you want to be part of it?

Working hours are during the day, Monday to Friday. Evening and weekend shifts may occur.

QUALIFICATIONS

The position requires that you are a licensed nurse or a further educated district nurse with the eligibility to work within Swedish healthcare. Previous experience as a diabetes nurse and/or experience from working in primary care is meritorious. Swedish language skills at least level C1 according to the Council of Europe scale are a requirement. Since travel occurs in the position, a valid B-driver's license is a requirement.
