Are you a confident and relationship-building person who enjoys meeting people and helping others find their way to community and support? Do you want to work proactively and preventively in an independent and operational role where you can be out in society and make a difference for the elderly residents of Nacka Municipality?
We are now looking for an Elderly Coordinator with an extended assignment as a Senior Guide in a pilot project, where you will become part of an experienced team within the preventive activities.
Your Assignment
In this role, you work proactively to reach elderly people who are at risk of loneliness or who have not yet found their way to the support and social contexts available in the municipality. The work takes place out in different parts of Nacka, where you move between meeting points, libraries, cultural houses, and other natural gathering places.
The role is about building trust, establishing relationships, and helping people move forward to the right interventions, activities, and community. In some situations, this also involves providing temporary and practical support to get started and find their way into new social contexts and activities.
You work both motivationally and guidance-oriented, becoming a key link between seniors, health centers, associations, civil society, and various operations within the municipality. The role also involves close collaboration with colleagues within the preventive activities.
The role includes, among other things:
- providing information about the municipality's elderly care, preventive activities, and activities within association life and civil society
- guiding and providing practical help to steer seniors towards the right interventions, activities, and contact channels
- working systematically proactively to reach elderly people who do not seek support or social contexts themselves
- having regular phone contacts and relationship-building follow-ups
- collaborating with health centers, social secretaries, associations, and civil society
- contributing to the development of working methods and finding new ways to reach the target group
- participating in or leading groups and activities as needed
The work is primarily during daytime, but some activities may occasionally take place in the evenings and on weekends. The position is based at Nacka City Hall, but the role is mainly mobile and outward-facing, where you are expected to be present out in the municipality and move between different meeting places and operations.

We are looking for someone who is confident in themselves, communicative, and has an easy time building trusting relationships. You enjoy meeting people in various contexts and feel comfortable in a role where you get to be visible, accessible, and build networks and collaborations, both out in the municipality and together with other operations and actors.
You are proactive and independent and appreciate freedom with responsibility. The role suits you if you like an operational and proactive job where you drive contacts and activities forward yourself and find ways to reach people who otherwise risk falling into involuntary loneliness.
To succeed in the role, you need to be resilient and motivated by long-term relationship-building and motivational work. You understand that change and motivation sometimes take time and can maintain energy and engagement even when results are not immediately visible. You have a good attitude, high service awareness, and the ability to create security and motivation in interactions with others. At the same time, you are curious, solution-oriented, and dare to try new working methods to further develop the assignment together with your colleagues.
Your Experience
Requirements
- University education such as social worker, nurse, occupational therapist, physiotherapist, behavioral scientist, or other education that the employer deems relevant
- Knowledge of aging and the living conditions of the elderly
- Interest in preventive work and the target group of the elderly
- Good digital competence
- Very good ability to express yourself in Swedish, both orally and in writing
Meritorious Experience

The position is a project employment for 18 months, starting as agreed, linked to a development assignment of strategic importance for the operations. Our organization works long-term with competence supply.
