We are an engaged and multiprofessional team operating in Norrtälje Municipality, and we are now looking for a Social Worker/Counselor on a 40-60% basis.
What We Offer YouAs a Social Worker/Counselor within ASIH (Advanced Support in Home Healthcare), you play a vital role in the multiprofessional team, focusing on psychosocial support for patients and their relatives. The work is based on the patient's situation and needs, often in connection with serious illness and end-of-life care.
There is also a full-time colleague counselor whom you will work closely with.
The position offers continuous supervision with other ASIH counselors, network meetings, further education, and an annual wellness allowance of 3,000 SEK, among other benefits.

We are looking for you who have a degree in Social Work. It is preferred that you are a licensed healthcare social worker and have a few years of experience within healthcare, palliative care, or ASIH, which is particularly meritorious. Your main task is supportive conversations including crisis support and existential support. You also support with practical and social legal issues, such as contact with authorities and applications for guardianship, fund resources, and transport assistance. Conversations with patients and relatives take place through home visits, clinic visits, or remotely.
You need to be able to work both structured and flexibly when needed. The role is primarily independent, but you will also have close collaboration with the team, consisting of doctors, nurses, nursing assistants, occupational therapists, physiotherapists, dietitians, and coordinators.

- Have a good ability to work independently and structure your work
- Are empathetic and receptive
- Have good collaboration skills in multiprofessional teams
- Degree in Social Work
- Category B driver's license and driving experience
- Experience with professional conversations
- Licensure as a healthcare social worker
- Authorization as a social worker
- Basic education in psychotherapy (so-called Step 1)
- Family Talk Intervention, FTI
- Other relevant courses or education.
