The job we offer is a permanent position at 100% starting 20260502 or by agreement. The working hours are primarily daytime Monday to Friday, but you will plan your own working hours based on the needs of the citizens.
In the role of care and support coordinator, you have a coordinating and collaborative function for adults with mental health issues or mental disabilities. You will work based on the case management model of integrated psychiatry. You will be responsible for quality work and ensuring evidence-based practices. You will meet citizens on-site or in their homes, and your workplace will be linked to the open meeting point activities centrally in Hässleholm. The role of case manager also includes mentoring staff on cases based on the working method. In combination with the assignment as care and support coordinator, you will support the meeting point activities and help develop preventive support based on the new social services act.

Who are you?
You who are applying have a support pedagogy education, social pedagogy education, or other relevant post-secondary education that the employer assesses as equivalent. You have several years of experience working in social psychiatry. Education and/or experience in case management and working with Independent Living, Motivational Interviewing, and Clarifying Pedagogy is an advantage. A valid driver's license (class B) is required for the job.
In your work, you show respect for the individual with a good attitude and understanding of other people's needs and feelings. You can take responsibility for your tasks, are flexible and proactive, and have a good ability to create good relationships and collaborate with others. Since the tasks place high demands on the ability to absorb and convey methods and working practices as well as quality work, you need to have good knowledge of Swedish, both spoken and written, as well as good computer skills. You need to have a good ability to collaborate with all staff categories and maintain good collaborative contacts.
