JOB DESCRIPTION

As a Case Manager, you will work closely with individuals with comorbidity, often in the borderland between mental health issues, harmful use, and social vulnerability. The task involves creating structure, coordination, and long-term solutions in situations where many actors are involved.
The work is based on a person-centered and coordinated approach supported by Signs of Safety. You will have an active and outreach role where you, together with the individual and relevant partners, work towards increased stability, independence, and participation.
The assignment may also include work at our meeting point for individuals with mental health issues and/or harmful use.
The position is part of our ongoing development in the field of social psychiatry and comorbidity.
Responsibilities
- Coordinate efforts around individuals with comorbidity
- Work motivationally and relationship-building in individual-focused work
- Support the individual in contacts with care, authorities, and other actors
- Contribute to structured collaboration internally and externally
- Participate in the development of working methods in the field of comorbidity
- Document according to applicable legislation
- Participate in the activities at the meeting point if necessary
QUALIFICATIONS
We are looking for someone who
- Has a post-secondary education relevant to the field
- Has experience working with mental health issues and/or addiction problems
- Has a good ability to work relationship-building and persistently
- Is structured but flexible in complex cases
- Thrives in collaboration between different stakeholders
Meritorious
- Experience in Case Management, FACT, ACT, or similar methods
- Experience in outreach work
- Knowledge of comorbidity issues
- Experience from municipal social services and/or psychiatry
