About us
Närhälsan's health centers in Hisingen have a varied number of registered patients of different ages.
The health centers offer a wide range of clinics for diabetes, asthma/COPD, smoking cessation, hypertension, as well as elderly and dementia care. We have several child health centers connected to our health centers. We have a rehabilitation coordinator and a psychosocial team consisting of psychologists, social workers, and care coordinators.
The operations are continuously developed to provide good care and maintain a good work environment for our employees. Around elderly patients, we have a good collaboration with the municipality and are responsible for doctors in municipal housing and patients receiving home care. About the job
In your role as a nurse with coordination responsibility, you will be responsible for the admission and discharge process when patients are admitted to and discharged from the hospital. In the autumn of 2024, we started a joint effort with municipal primary care, assistance units, and specialist and inpatient care to increase collaboration and create higher quality for patients and relatives. In 2026, we will implement the new regional routine for admission and discharge. The work should provide good conditions for collaboration between all parties and strengthen continuity, patient safety, and a common working method.
You will work from a location in Sannegården and, together with colleagues, be responsible for patients registered at Närhälsan's health centers in Hisingen who need individual collaboration during admission and discharge. The process follows regional guidelines, and you will work in the IT service SAMSA.
The work consists of administrative tasks and care planning meetings. An important part is home visits and contact with patients. In primary care's assignment, it includes establishing and following up SIP (Coordinated Individual Plan), at the hospital, short-term care facility, or in the patient's home. You may also become the patient's primary care contact.
About you
You are a licensed nurse with experience from health centers and/or home care. It is an advantage if you have worked with the admission and discharge process and previously had responsibility for SIP. Experience with the IT service SAMSA and the discharge process from inpatient care is also an advantage.
We are looking for someone who takes initiative and is confident in making their own decisions, as well as enjoys collaborating with various stakeholders. You are meticulous and have a problem-solving ability where you can effectively coordinate and prioritize your tasks. We believe you enjoy teamwork and see it as natural to collaborate with both colleagues and other contacts outside the organization.
You meet many patients, so it is important that you are accommodating, attentive, and have a strong commitment to helping others. You express yourself well in speech and writing and are a good listener, both in digital meetings and during visits to the clinic. We also hope you have an interest in digitalization and business development.
