Rehabilitation Coordinator at Sigtuna Läkarhus
We offer you a part-time position as a Rehabilitation Coordinator at our clinic, where your role is to support our patients in the sick leave and rehabilitation process.
About Sigtuna Läkarhus
Sigtuna Läkarhus is located on the picturesque pedestrian street in beautiful and charming Sigtuna, 30 minutes from Stockholm. It is easy to reach the health center by bus, bike, or car.

We are 20 employees who provide primary care for about 5800 listed patients. Within the organization, we offer medical and district nurse consultations, home care, telephone advice, diabetes clinic, asthma/COPD clinic, elderly care, psychological consultations, and child health care.
Now we continue to develop care in the outskirts to support urgent care, work preventively, and reduce readmissions to acute hospitals. Our quality work for 2024 and 2025 is directed towards our home care.

Job Responsibilities:
Your responsibilities include, among other things: Motivating and informing identified patients about interventions that counteract sick leave when necessary. Assessing the patient's rehabilitation needs. Mapping the patient's ability to work and the demands of the job. Identifying/coordinating rehabilitation efforts in consultation with the treating physician and other health professionals. Establishing an individual rehabilitation plan and return-to-work plan. Contacting the patient's employer.
Being the contact person for the patient with the Social Insurance Agency, healthcare, the Employment Service, and other parties. Coaching the patient with the aim of returning to work. Being an advisor and knowledge provider about rehabilitation and sick leave at the health center. Compiling and analyzing sick leave statistics.
Qualifications:
Licensed health and medical care professional or social worker with several years of experience in rehabilitation. You have completed the course "Coordination of the Rehabilitation Process, 7.5 credits at KI or equivalent knowledge of insurance medicine.
Experience in rehabilitation is an advantage. Good IT skills, advantageous if you are proficient in Excel. MI competence.
Employment Form:
Part-time employment. Possibility of extension to full-time employment exists. Working hours Monday to Friday. Start date according to agreement.

You have a good empathetic ability and personal maturity, which gives you independence and contributes to your responsibility for the patient, yourself, and your tasks. You plan, organize, and prioritize work effectively. You set and adhere to deadlines. We are looking for someone who wants to help develop our operations with a patient focus!

- in addition to Swedish professional licensure, good knowledge of the Swedish language corresponding to level C1 according to the Common European Framework of Reference (CEFR) is required. The same requirements are guiding for non-licensed personnel with patient contact.
