The health center Centrumkliniken has about fifty employees serving approximately 10,200 registered patients. We are responsible for two nursing homes (SÄBO) and have an associated child health center (BVC) at a four-legged Family Center. In addition, we have a large rehabilitation unit, Lyftet Rehab, with physiotherapists and occupational therapists. Our health center has a good and creative combination of experience and innovation in the staff group, and we conduct a variety of clinics, such as diabetes, asthma/COPD, blood pressure, smoking cessation, refugee health, district nurse clinic, elderly care clinic, and memory clinic. At Lyftet rehab, a number of group activities are also conducted, such as Osteoarthritis School, balance group, pool training, training for new mothers, and more. For over a year now, we have been working based on the model of continuity and teamwork, a working method aimed at strengthening and developing our team collaboration and providing good care to our patients. The work includes team rounds twice a week, where each team is responsible for its patients.

At our health center, we also have a very well-functioning psychosocial team with two social workers and a rehabilitation coordinator, which includes structured work around sick leave. The team also offers CBT and group treatments such as Mindfulness and anxiety groups. We have a very close collaboration between the different professional categories, and we are proud of our open, positive, and creative work climate.

This spring, we were the first in Skåne to become an antibiotic-smart health center, and this is one of the areas we are working on systematically to increase medical quality and patient safety.

JOB RESPONSIBILITIES

We offer a workplace where we care for each other and constantly drive development work. Our patient clientele is varied, and a workday with us is never the same, as you work with both planned and acute doctor visits. Additionally, home visits and the implementation of SIP according to the law on cooperation at discharge are included. There is an opportunity for on-call duty at our joint evening and weekend clinic in Trelleborg. We invest in continuing education and professional development and are flexible in allowing you to influence and shape your job content.

You will have close cooperation with our various professional categories, and we offer a stimulating, pleasant, and positive workplace.

The position is a temporary contract for 4-6 months with the possibility of extension.

QUALIFICATIONS

You who are applying are a licensed physician or specialist in general medicine with the authority to practice within Swedish healthcare. Language skills in Swedish at least level C1 according to the Council of Europe’s scale are a requirement. Previous experience in primary care is an advantage, as is experience from Hälsovalet. If you have worked in our systems PMO and Mina Planer, it is a benefit for the position.

We are looking for someone with good collaboration skills who sees the importance of our patients receiving a good response. You have a great interest in general medicine and the patients you meet in primary care. We believe you are attentive, service-oriented, flexible, and meticulous, all to provide good and qualitative care to our patients.
