About the operation:

The Health and Medical Care Unit (HSE) of Vallentuna Municipality is an independent operation for licensed personnel within LSS. We sit together in bright and pleasant premises at Väsbygården's elderly care home and have a close collaboration with the municipality's other licensed personnel in elderly care.

We are responsible for healthcare at the municipality's LSS homes and daily activities as well as the social psychiatry support housing. The area consists of 7 housing units, including one social psychiatry unit, 2 daily activities, and a children's and youth home. The HSE team consists of a unit manager, nurses, physiotherapists, and occupational therapists.

The Social Administration consists of about 500 employees with the task of working preventively and supportively within both social services and care. The operation consists of secure workgroups with helpful colleagues and supportive management. Within the Social Administration, you as an employee have great freedom and trust to perform your work and the opportunity to develop your role.

Job Responsibilities:

The physiotherapist's main responsibility lies with LSS homes and daily activities as well as social psychiatry. Responsibility within special housing for the elderly and work with preventive measures will also be included in your tasks.

As a physiotherapist, you are overall responsible for leading the municipal health and medical care assignment within the area of social psychiatry/LSS. The assignments are carried out as home visits or delegated tasks to care staff. Different patients with varying medical conditions place varying demands on your competence, and you build up a broad knowledge.

Being a physiotherapist is an independent job where you need to be secure in your professional role. Your workday may involve visits to several units to provide support or conduct assessments.

As our physiotherapist, you are responsible for assessments of function, fall risk, and activity ability. We work with maintaining functions and fall prevention measures, and if necessary, set goals and treatment measures with delegation to staff. Fitting and prescribing aids are also part of your tasks. Your responsibilities include guidance, training, and information, e.g., transfer techniques and handling of aids, to care recipients and care staff. The role also includes participating in team meetings with other professionals.

Your assignment includes actively working with function-maintaining rehabilitation within the health and medical care's areas of responsibility. Collaborating and communicating with both internal and external actors is essential. You will work with deviations and documentation, guide, delegate to, and teach staff in health and medical care interventions, as well as identify and prevent risks in care and map the patient's needs for health and medical care interventions with an emphasis on person-centered care.

Qualifications:

  • You are a licensed physiotherapist with at least 2 years of experience in primary care and/or home healthcare.
  • A driver's license (B) is required.
  • Proficiency in the Swedish language in both speech and writing.
  • You need to have good computer skills as all documentation is done in digital systems. We use Office 365.

It is meritorious if:

  • You have experience meeting people with disabilities within LSS and social psychiatry.
  • You have experience guiding care staff.
