As a social worker with us, you will work with patients and relatives in both geriatric inpatient care and ASIH. You will provide psychosocial support, counseling, and advice while collaborating with other professionals in the team to strengthen the resources of patients and their relatives in a complex care situation. Responsibilities include conducting assessments, participating in care planning, and collaborating with external actors such as authorities and primary care. You will be an important part of the multidisciplinary team, focusing on the best interests of the patient.

• Degree in Social Work or another equivalent higher education deemed suitable for social work in healthcare by the National Board of Health and Welfare.
• Experience working as a social worker in healthcare, preferably in geriatrics, palliative care, or ASIH.
• Good ability to work independently and make decisions based on the organization's routines and relevant laws and guidelines.
• You express yourself well in spoken and written Swedish. Additional language skills are an advantage.

• You have a stable demeanor and approach patients, relatives, and colleagues with confidence and professionalism.
• You are collaborative, open, and friendly, actively contributing to a positive work environment where differences are seen as an asset.
• You are solution-oriented and have the ability to see opportunities even in challenging situations.
• You are considerate, attentive, and have an understanding of the patients' entire life situation.
• A valid driver's license is required.
