Coordinator/Social Worker (or equivalent) for the VUB Team

The VUB team is a unit within Habilitation & Health, a service within Region Stockholm with special expertise in disabilities.

The unit specializes in intellectual disabilities and severe challenging behaviors, receiving patients from the age of 12 and up. The team is part of a comprehensive solution that serves as an enhancement beyond the regular healthcare offerings. We have three professional categories working as therapists: psychologists, special educators, and psychiatrists. In addition to these, there is a unit manager and a coordinator. The interventions are time-limited and directed towards the patient and individuals in the patient's network.

About the position:

As a coordinator in the VUB team, you will truly be the spider in the web. You have an overview of where we are in our process for each individual patient, and you maintain continuous contact with the patients' representatives and networks. You make the initial contact with representatives for new patients, schedule visits, and help drive the cases forward. With your knowledge and experience in LSS and SoL, you will be a valuable sounding board for both the therapists and the patient's relatives. This also means that you have the terminology and knowledge to conduct the important dialogue with responsible parties both within the municipality and with our healthcare neighbors. You actively work to achieve good coordination between the region's and municipality's actors, both at the individual level and at a general level (together with the VUB team's unit manager). Furthermore, you are responsible for planning and inviting networks to SIP and will often have an active role in these meetings. Administrative tasks are also part of the job, such as scheduling, bookings, handling invoices, and some personnel administration.

We are looking for:

We are looking for someone who is a trained social worker or has equivalent experience. You are accustomed to working based on LSS and SoL, and have a good understanding of the intentions of the laws. You feel comfortable in the coordinating role that the position entails and can quickly gain an overview of a patient case. As a person, you are pedagogical and secure in your communication, and you easily establish trusting contacts. Teamwork is something you value. You have good computer skills and can easily familiarize yourself with new systems. You speak and write fluently in Swedish, and it is an advantage if you can also hold simple conversations in English. We would like you to have experience with the TakeCare journal system.

• employees who will have patient contact must undergo a new hire health examination, according to current directives, which is conducted through occupational health services before employment.
• in addition to Swedish professional certification, good knowledge of the Swedish language at level C1 according to the Common European Framework of Reference (CEFR) is required. The same requirements are guiding for non-certified staff with patient contact.
