About 45 employees work here in interdisciplinary general medicine teams to provide our approximately 14,000 registered patients with good, close, and accessible care.
Key actors for the patient collaborate through teamwork, where continuity is our cornerstone.
The health center is characterized by engaged and solution-oriented employees who, with professionalism and knowledge, take responsibility for both the development of the organization and the daily work with the patient in focus. We drive improvement initiatives initiated by employees to create more efficient care processes, better continuity, more person-centered and accessible care, along with a sustainable work environment. The patient-centered work is based on medical assessments and priorities, where we work with evidence-based treatments. We actively work to develop our treatment efforts for increased psychosocial health, good living habits, and to prevent long-term sick leave while promoting a quick return to work.

Your Responsibilities
With us, you will work both independently and in a team around the patient. The work is varied, broad, and educational, where we utilize your skills, experiences, and areas of interest based on the needs of the patients.
You will work within a broad generalist area and use your knowledge to meet and care for patients of all ages, with a wide variety of care needs, disease profiles, and functional abilities. You will have close collaboration with your nursing colleagues and other professionals at the clinic. You will handle both planned and acute visits, coordination, and should be able to adapt when re-prioritizations need to occur. Within the group, there are specialist competencies in lifestyle habits, diabetes, heart failure, asthma/COPD, osteoporosis, and incontinence, where we strive to use competencies as effectively as possible in patient-centered work.
Your responsibilities largely consist of telephone counseling and clinic work. You will make assessments, triage, and perform investigative, treatment, nursing, and medical tasks. You are responsible for assessing care needs, triaging, and providing advice via telephone through TeleQ, as well as managing digital care cases via the 1177 e-service.
Clinic work includes tasks such as vaccinations, medication administration, examinations, and wound dressings. You coordinate, plan, and perform nursing and medical actions in accordance with the patient's care plan. In some cases, you will act as a primary care contact for patients needing coordination, where you play an important role in the team around the patient based on individual needs and care plans.
Who are you?
You are a district nurse or a licensed nurse. We prefer that you have experience in telephone counseling and coordination of patients' care needs.
You want to work with us to provide good care of high quality, and you find it easy to create good meetings with both patients and colleagues.
We place great emphasis on your personal qualities and expect you to take responsibility and contribute with engagement and joy at work. As a person, you are engaged, solution-focused, and responsible.
You communicate clearly both orally and in writing and have good knowledge of the Swedish language corresponding to level C1 according to the Common European Framework of Reference (CEFR).
