Social Worker Assistant for SVU Team

We are now looking for a dedicated social worker assistant for a longer temporary position as one of our SVU caseworkers will go on parental leave in April.

Your mission

As a social worker assistant in the SVU team, you will work with the individual in focus and strive to strengthen the individual's capabilities.

Your main responsibilities include:

• Assessing the individual's needs for help and support after hospitalization, including medical, practical, and social aspects.
• Coordinating and planning necessary interventions, such as home care, rehabilitation, or aids, to support recovery at home.
• Collaborating with other professionals to ensure that all aspects of care are considered.
• Providing information and support to our citizens and their relatives about available resources and services.
• Following up to ensure that interventions are working as planned and making adjustments as needed.

Through these tasks, the social worker assistant ensures a safe and effective transition home, promoting the individual's well-being and independence.

Your experience

• Experience working in home care or residential care.
• We prefer that you have previously worked with regulatory authority as a social worker.
• Familiarity with working in Lifecare.

Is this you?

• Swedish social worker degree.
• Valid driver's license (manual transmission).
• Good knowledge of the Swedish language.

We consider it an advantage if you have:

• Experience in a role as a social worker assistant and/or within regulatory authority.
• Worked in home care or residential care.

The Department of Care and Support encompasses four regulatory activities: social services for those over 18, financial assistance, elderly care, and support for people with disabilities. With the individual’s needs in focus, we work to achieve good living conditions for residents in need of care and support and for their participation in community life.
