We are looking for a Case Manager for Linköping's Support Housing. 

About Linköping's Support Housing The operation is run by Nytida on behalf of Linköping municipality and includes two units in central Linköping with a total of 28 places. The units provide round-the-clock support to individuals with long-term dependency issues, where mental health problems and illness may also occur. The aim is to provide support focusing on rehabilitative and supportive measures to give individuals the best possible conditions to eventually manage ordinary housing. The work includes networking and collaborating with key stakeholders to create a holistic approach and the best conditions for individuals to maintain sobriety. 

About the role   We are looking for a Case Manager for our support housing who works Monday to Friday coordinating efforts around the tenants in each residence. As a Case Manager, you will be responsible for;

  • mapping needs and current status of the individual using validated methods
  • having good knowledge of the individual and focusing on daily life, employment, constructive networks, family, leisure, and need for care
  • establishing an action plan with set goals and sub-goals in consultation with the individual and their contact person 
  • coordinating efforts from various stakeholders such as social services, psychiatry, addiction care, insurance agency, and primary care
  • working with the unit manager to find partners and networks that can benefit individuals in the support housing
  • continuously evaluating the efforts
  • being part of the daily work at the units and with the individuals

Your experience and knowledge You who apply should have a higher education degree in social work or a behavioral science subject and experience with the target group. 

A driver's license is a requirement for the position.
