Are you the right person for our new role as a health coordinator? We are looking for someone who is passionate about good, equal, and safe healthcare, for and together with our residents! In Region Västernorrland, there are three primary healthcare areas; south, west, and north, where local collaboration takes place between municipalities, primary care, and specialist care. Together, we work to create a cohesive care chain for the growing patient groups of chronically ill and multi-ill. The Western primary healthcare area has a hospital, five regionally operated health centers, and Lifestyle Medicine Österåsen. The area includes the two municipalities of Sollefteå and Kramfors. Within the Region, we strive to achieve our vision of "life, health, and sustainable development in Västernorrland". In the work with "Good and Close Care", primary care in the West and Kramfors municipality are now making this joint effort.

We are now looking for a Health Coordinator for the Kramfors area. The position is a project employment for 2 years with entry as agreed.

Job Responsibilities

In the role of health coordinator, you are expected to be the link between residents and the healthcare system's range of services, where you work to maintain and strengthen residents' mental, physical, and social well-being. The work is aimed at all residents of the municipality.

As a health coordinator, you have a planning and coordination responsibility to, in good dialogue with our healthcare neighbors like the municipality and specialist care, carry out health promotion initiatives in collaboration with the population. This could include, for example:

• initiatives in the areas of diabetes, cardiovascular diseases, and stress
• increasing knowledge about digital healthcare meetings, self-care, and navigation within healthcare.

The work is done independently where you are responsible for developing initiatives from planning to implementation. The tasks also include regularly mapping, analyzing, implementing measures, and following up on health promotion work. You are expected to investigate and assess the needs of residents, and expert colleagues contribute with information, dialogue, and education within their professional field where appropriate to meet residents' unique conditions and address their needs.

Qualifications

We are looking for someone who

• Has post-secondary education preferably in public health
• Has good knowledge of Swedish in speech and writing
• Has a B driver's license

It is meritorious if you

• Have work experience in primary care
• Have previously worked in a coordinating/moderating role
• Have previous experience in project work
