The Social Psychiatric Addiction Team, SBT, is a dual diagnosis team where a group leader, two social workers, and three CM/rehabilitation assistants work closely together to meet clients' needs for support according to the Social Services Act and/or LVM. SBT works with the target group of adults with mental health issues and addiction problems. A large part of our daily work involves meeting and guiding people who are in a difficult stage of life. As a unit, we work rehabilitatively, and our goal is to free and develop the individual's resources. We are keen to strengthen what works for clients, and we want our approach to be perceived as inclusive and flexible.

In our organization, you will meet engaged, creative colleagues who provide housing support and addiction treatment based on an individual plan with the client in focus. We depend on each other – it is with our combined expertise that we can determine what is best together with our clients. We are now looking for a CM/rehabilitation assistant to join our team. An important task involves coordinating and guiding the client, directing them to the right agencies, motivating lifestyle changes, and being a support in various contexts.

SBT is in the midst of a development where we will focus more on working methodically and structurally, and we will invest in training within social work.

Your tasks

The position is a rehabilitation assistant, but the tasks resemble what is usually described as Case Manager. You have a coordinating function with responsibility for planning and coordination. You provide housing support and, in some cases, treatment. The support is tailored to the individual's needs and opportunities to develop a functioning everyday life. Since needs and wishes vary greatly, the support is individually designed for each person. Implementation plans and crisis plans are central parts of your work with the client. We work according to R-ACT, and therefore it is important that you appreciate variation in everyday life and a lot of collaboration.

Who are you?

Requirements:
- Minimum two-year full-time education as a mental health worker, social educator, or other education deemed equivalent by the employer
- B driver's license
- Good communication skills in both spoken and written language
- Experience working with adults with addiction problems and mental health issues (dual diagnosis issues)
- Basic computer skills and experience with social documentation
- Experience in collaboration and coordination between social services and the region
- Knowledge of social services and the laws and regulations governing the municipality's and region's efforts for the target group
- Good knowledge of society and the welfare system
- Knowledge of Motivational Interviewing (MI)

Meritorious:
- Training in evidence-based methods such as ASI, CRA, Relapse Prevention, etc.
- Training in CM method/R-ACT

The Social Psychiatric Addiction Team consists of curious colleagues with a strong commitment to the target group. We are a safe workplace where we care about continuously developing our work. The unit applies annual working hours. SBT is centrally located in Södertälje, where we share premises with outpatient care, the domestic violence center, and the Authority for Harmful Use and Dependence.

We are now recruiting for a permanent position with working hours during the day, Monday to Friday.
