The Authority Department, unit SVU, is now looking for a physiotherapist to our fantastic team while a colleague is on parental leave!

We are a team that enjoys working together. We put the team before the individual and we all help each other in our cases to get a holistic view of the user. The team you will be working in consists of 10 case managers, 5 nurses, 3 occupational therapists, and 3 physiotherapists, all of whom possess different experiences that enrich the group.

Job Responsibilities

As a physiotherapist in the care planning team, you are responsible for a safe and secure transition of patients from inpatient care and follow-up during short-term stays. You create conditions for individuals to live as independently, participatively, and meaningfully as possible based on a person-centered approach with a focus on the healthy. Together with your colleagues, you plan for discharge, which may involve digital planning, personal meetings at the hospital, or phone/Skype planning.

The work also involves making assessments and reporting based on HSL to other operations in connection with discharge from the hospital and short-term accommodation. You will need to collaborate in a team with other professional groups both internally and externally. It is required that you have good contact with relatives and good teamwork with nurses and case managers to create conditions for providing users and relatives with security and good service.

Qualifications

We are looking for someone who:

• Is a licensed physiotherapist
• Has previous experience as a physiotherapist in municipal operations and/or in inpatient care/primary care
• Can express themselves well in Swedish, both verbally and in writing
• Has good insight into applicable legislation, established goals, and guidelines
• Is computer literate

It is an advantage if you have experience in:

• SVU process and short-term process
• Interprofessional work
• Mina planer/Procapita

About the Workplace

The Authority Department in the Health, Care, and Social Services Administration is organized into three units, one of which is responsible for collaboration and planning for discharge from inpatient care and follow-up of short-term decisions.

In everyday language, we call ourselves SVU, an abbreviation for collaboration during discharge from inpatient health and medical care. The unit consists of about 80 employees divided into three sections with interprofessional teams including case managers, nurses, occupational therapists, physiotherapists, and assistants. The professions in the team decide based on their assignments on the need for assistance according to the Social Services Act (SoL) and interventions according to the Health and Medical Services Act (HSL).
