We work based on assignments from authorities and provide support to individuals with various disabilities living in their own homes throughout Haninge municipality. The interventions are carried out together with the individual. The support involves providing assistance to individuals with intellectual disabilities, neuropsychological disabilities, or social psychiatric difficulties on behalf of the commissioning unit.
We operate from centrally located premises in Haninge.
Your mission
You will have primary responsibility for around 20 clients and work based on objectives set by the authority - LSS, Social Psychiatry, or IFO Adult. The work is guided by orders we receive in Lifecare IBIC. An individual implementation plan is established together with the client, which you will follow up and modify based on how the support develops. Our main goal is to provide individualized support to individuals living in their own homes in the municipality for increased independence.
The content of the assignments varies from support in contacts with authorities, collaboration with networks, and structure in the home. As a housing supporter, you will be part of a team of about six other colleagues. You will mainly collaborate with three other housing support teams, group leaders, unit managers, and commissioning caseworkers, in addition to collaborating with the client's network as needed.
Currently, we have most assignments during the day, and some evenings may occur until 7:00 PM on weekdays.
We have a 40-hour workweek for full-time positions.
What we are looking for in you
- You have relevant education as a social pedagogue, support pedagogue, nursing assistant, or caregiver in psychiatric care with further training in NPF, ESL, MI, behavioral science, or a bachelor's degree in mental health with a focus on complex issues.
- You have previous experience with motivational interviewing, knowledge of low-arousal approaches, and understanding of how autism spectrum disorder affects individuals.
- You have experience in similar work.
- You should have good self-awareness. Since our clients may experience difficulties at times, it is crucial to remain calm and balanced. You have a solution-oriented empathetic approach and are positive.
- You have a professional attitude and can build a good relationship with the individual. It is important to support the client in their goals and sub-goals through motivational interviewing and to use various cognitive aids.
- You have a good ability to sense needs and create structure, propose solutions, and interact with the individual.
- You have a good ability to express yourself in speech and writing and are well acquainted with the Swedish language.
We place great importance on personal suitability and your ability to collaborate with other colleagues and partners.
A driving license is desirable.
