About Us

Närhälsan's health centres on Hisingen have a varied number of listed patients of different ages. The health centres offer a wide range of clinics for diabetes, asthma/COPD, smoking cessation, hypertension, as well as elderly and dementia care. We have several Child Health Centres (BVC) linked to our health centres. Here, a rehabilitation coordinator and a psychosocial team consisting of a psychologist, social worker, and care coordinator work together.

The operations are continuously developed to provide good care and maintain a good working environment for our employees. Regarding elderly patients, we have a good collaboration with the municipality and are responsible for doctors in municipal housing and patients with home care. Here you will have the opportunity to develop working methods for the admission and discharge process and work for good collaboration between primary care, the municipality, as well as specialist and inpatient care.

About the Job

In your role as a nurse with coordination responsibilities, you are responsible for the admission and discharge process when patients are admitted to and discharged from the hospital. In autumn 2024, we started a joint effort with municipal primary care, assistance units, as well as specialist and inpatient care to increase collaboration and create higher quality for patients and relatives. During 2026, we will implement the new sub-regional routine for admission and discharge. The work should provide good conditions for collaboration between all parties and strengthen continuity, patient safety, and a common working method.

You are based at a location on Sannegården and, together with colleagues, are responsible for patients listed at Närhälsan's health centres on Hisingen who need individual collaboration during admission and discharge. The process follows region-wide guidelines, and you work in the IT service SAMSA.

The work consists of administrative tasks and care planning meetings. An important part is home visits and contact with patients. The primary care mandate includes drawing up and following up on SIP (Coordinated Individual Plan) at the hospital, short-term care facility, or in the patient's home.

About You

You are a registered nurse with experience from a health centre and/or home care. It is meritorious if you have worked with the admission and discharge process and previously had responsibility for SIP. Experience with the IT service SAMSA and the discharge process from inpatient care is also meritorious.

We are looking for you who take initiative and are confident in making your own decisions, and who enjoy collaborating with different actors. You are meticulous and have problem-solving skills where you can effectively coordinate and prioritize your work tasks. We believe you thrive in teamwork and see it as natural to collaborate both with colleagues and other contacts outside the organization.

You meet many patients, so it is important that you are accommodating, sensitive, and have a strong commitment to helping others. You express yourself well in speech and writing and are a good listener, both in digital meetings and during visits at the clinic. We would like you to have an interest in digitalization and business development.
