Case Worker for Health and Social Care Administration in Köping Municipality

We are part of the Health and Social Care Administration, responsible for the municipality's services for the elderly and people with disabilities. The administration has approximately one thousand employees.

The unit handles assistance administration according to the Social Services Act (SoL) and the Act on Support and Service for Certain Persons with Disabilities (LSS), as well as transport assistance administration and the implementation of contact persons and support families. We work with needs assessment using the models IBIC (Individual Needs in Focus) and ICF (International Classification of Functioning, Disability and Health) to highlight the user's needs. Our current documentation system is Life Care. In your daily work, you will receive support and supervision from the unit manager, team leader, and colleagues.

At our workplace, every employee is offered the opportunity to work remotely up to two days per week, flexible hours, free staff parking, and a wellness allowance.

Job Responsibilities

As a case worker here, you are part of important and meaningful work. You investigate and decide on measures according to the Social Services Act. The work also includes follow-up and review of previously decided measures. The role involves dialogue with individuals seeking measures and/or their relatives. The job also entails contact with other authorities and organizations. Collaboration takes place with our executors and other relevant partners, both internally and externally.

In the case worker group, we share responsibility for developing operations, working with quality issues, contributing to a good work climate, and ensuring a strong user perspective. To succeed in this role, you must have the ability to gather information, compile, analyze, and evaluate different action alternatives to make assessments and propose measures. Therefore, you need good empathy, flexibility, and a forward-thinking mindset to find the right individual solutions. You should be able to structure and document your work easily and have good communication skills.

Qualifications

We are looking for you who have a degree in social work, a social care program, or another academic education that the employer considers equivalent. You must have knowledge of current legislation, regulations, and guidelines in the field. You should also have knowledge of investigation and case handling regarding measures. Previous experience is meritorious. Since home visits are part of the job duties, a manual B-driver's license is required as our users live throughout the entire geographical area of the municipality.
