DocCura Health Centre is a newly established clinic that has quickly become a appreciated alternative for patients in the expanding Järfälla municipality and surrounding areas.

We are looking for you who want to be a key person in a cross-professional team at our health centre in Järfälla.

As a rehabilitation coordinator, you will be involved from the beginning to shape collaboration, routines, and rehabilitation efforts – always with the patient at the center.

DocCura Health Centre is a newly established clinic in a growing area in Järfälla municipality, commissioned by Region Stockholm. Our goal is to offer the best care and accessibility for our patients. About the role:

You are responsible for coordinating and following up on individualized rehabilitation plans for patients who are on sick leave or at risk. The work is carried out according to Region Stockholm's guidelines and includes:

• Assessment of rehabilitation needs
• Coordination between healthcare, employers, the Swedish Social Insurance Agency (Försäkringskassan), and other actors
• Medical record keeping and use of KVÅ codes
• Close collaboration with doctors and psychologists in the team

We are looking for someone who:

• Is a licensed healthcare professional, occupational therapist, or social worker
• Has experience in coordination or guidance regarding sick leave and rehabilitation
• Has several years of experience in rehabilitation and has completed the Karolinska Institutet course "Coordination efforts - coordinating the sick leave & rehabilitation process (7.5 credits)" or has equivalent knowledge of insurance medicine
• Thrives in a team but is also self-motivated
• Wants to help shape a healthcare offering from the ground up
