The Social Services Administration is responsible for providing support, care, and assistance to people at different stages of life. We work based on individual needs and offer interventions grounded in several laws, including the Social Services Act (SoL), the Act on Support and Service for Certain Persons with Functional Impairments (LSS), and the Health and Medical Services Act.

Responsibilities
We are looking for you who want to work as a social secretary in the Care Planning Team. The team's mission is to be a strong and natural partner in collaboration with inpatient care and primary care. The individual's path through the discharge process should be as efficient as possible, with the individual's best interests always in focus.
Previous experience in authority decision-making, the discharge process, and working in the LifeCare system is advantageous. We place great emphasis on personal suitability, particularly your ability to collaborate, create structure, and communicate clearly in both speech and writing.
The role includes, among other things:
- Investigating the need for support in connection with hospital discharge and planning for a safe return home.
- Collaborating with the region, primary care, and municipal primary care during admissions and discharges based on the individual's needs.
- Short-term follow-up. The Planning Unit follows up short-term decisions until the person leaves short-term care or is granted care and nursing housing.
- Active collaboration with other professions to achieve a safe return home.
Qualifications
To thrive in this role, you need to be confident in your professional competence, have a good attitude towards people, and be able to work both independently and together with others.
You have a structured approach to work, planning, organizing, and prioritizing tasks efficiently. You ensure that work is completed within the set timeframe and follow up on results to achieve set goals.
You have a professional and friendly attitude, are service-oriented, and show respect in all contacts, even when the pace is high.
You are flexible and adaptable, easily adjusting to changed circumstances, and see opportunities in change. At the same time, you are sensitive and have a good ability to understand and take into account the needs, feelings, and perspectives of other people.
Requirements for the position:
- Social Work degree (Social Worker/Socionom)
- Very good ability to express yourself in Swedish, both orally and in writing
It is advantageous if you have experience in authority decision-making according to the Social Services Act, documentation in accordance with current legislation, and knowledge of the admission and discharge process from inpatient health care.

We offer a temporary position at 100% during the period September 1, 2026 – September 1, 2027.
