The Home Plan Team is expanding its family treatment staff.
Job Responsibilities
The Home Plan Team offers intensive and flexible family treatment focused on enhancing parental capacity for families with children and adolescents exhibiting norm-breaking behavior. The target group consists of families with children between the ages of 10 and 18.
We work with a specific target group: children and adolescents with norm-breaking behavior and their immediate network. This may include youth with acting-out behaviors, high conflict levels at home or in other settings, associations with older criminals, presence in areas linked to drug sales, or a risk of developing criminal behavior or thought patterns.
The goal of the intervention is to help families find functional ways to manage their difficulties and develop effective communication channels. The Home Plan Team collaborates closely with "Lyftet" and "Flexet," two other sections within the Platform unit. While Lyftet or Flexet often handle the support and treatment for the child/adolescent, the Home Plan Team focuses on treatment for the parents and other key figures in the child's life.
Treatment is primarily conducted in the home environment. At least once per term, the Home Plan Team offers parenting courses based on the Connect model. The work schedule is 08:00–16:30, with opportunities for weekend work. Documentation is done in Journal Digital.

Since you will meet many people daily, including adolescents, their families, colleagues, and other actors, you must have strong collaboration skills and the ability to build relationships. As a significant part of the role involves coordination and contact with multiple actors, you must be structured and organized. Flexibility regarding working hours, changing circumstances, and a willingness to test new working methods are also important.
About the Workplace
The Home Plan Team is an outpatient service within the Employment and Social Administration, organized under IFO Intervention, Platform Unit Malmö, Support and Treatment. The section includes a section manager, a coordinator, and nine family therapists.
