RESPONSIBILITIES

Take the chance to be part of launching our home discharge teams within the Social Care Administration!

In the role of occupational therapist, you belong to Kommunrehab but are part of the Social Care Administration's home discharge team together with a physiotherapist, nurse, and case worker.

As an occupational therapist, you will work with the team to plan for the patient to have a safe and secure transition home. Your main responsibilities include:

• Initiate and follow up on discharge planning at an early stage
• Assess the need for interventions and gather relevant background information
• Participate in discharge planning and clarify responsibilities and needs
• Collaborate and communicate with healthcare providers, patients, and relatives
• Monitor and communicate in LINK and coordinate with short-term accommodation regarding short-term stays.
• Prepare interventions relevant for home discharge in collaboration with the regular home care unit. The regular unit takes over responsibility for implementing interventions in connection with home discharge and also plans follow-up.

QUALIFICATIONS

We are looking for you who are a licensed occupational therapist and have good knowledge of collaboration between municipal primary care, regional primary care, and inpatient care. We place great emphasis on engagement and that you have good collaborative and planning skills.

You have a driver's license, master Swedish in speech and writing, and are accustomed to using a computer as a work tool. Personal suitability is highly valued.
