Primary Care Centre Östergötland consists of 33 region-run health centers, Vårdcentrum Finspång, the Child Health Care Unit, evening and weekend 1177 and emergency clinics, and the General Medicine Education Centre. Cityhälsan Centrum Health Center is centrally located on the old hospital grounds in Norrköping.

Here you will find a practice with approximately 14,100 registered patients in an average population and nearly 60 permanent staff members.

At our center, you work in interprofessional teams where collaboration and continuity are central. We have four larger teams at the health center and, since this spring, smaller teams where each doctor has their own nurse. This creates good conditions for close cooperation, continuity in patient care, and a working method where we utilize each other's competencies. With a stable basic structure in the practice, you also receive good support in your daily work.

We also have several specializations, including an elderly care clinic, and are an HBTQI-certified health center. This reflects our ambition to offer high-quality inclusive care and a workplace where different perspectives and needs are taken seriously. This gives you a varied daily routine, a stimulating pace, and close collaboration with many different professions.

We are an IBH health center (IBH = Integrated Behavioral Health), which means collaboration between doctors, psychologists, rehabilitation coordinators, and nurses, benefiting both patients and staff. The goal is to work together across the entire health center to increase the functional level and quality of life for many patients, as well as to enhance patients' ability to manage their ailments and avoid over-medicalization.

Job Description

We are looking for a Diabetes Nurse for a permanent position. As a diabetes nurse at the health center, you will run a diabetes clinic together with a colleague. A diabetes-responsible doctor is also part of the team. You manage diabetes patients at the clinic relatively independently and collaborate with the respective Patient Responsible Doctor (PAL). We strive for continuity regarding the patient's contact with both the doctor and the diabetes nurse. The work is a mix of physical clinic visits, telephone follow-ups, and administrative tasks. Participation in further education is encouraged.

As a nurse with us, the role also includes being a resource nurse, where the nurse, in collaboration with the resource doctor and resource assistant nurse, handles emergencies occurring at the health center. Two mornings a week, we have a minor emergency clinic staffed by a doctor and a nurse who, in collaboration, handle booked patients. It is possible that you will also work in one of these functions, which can contribute to variety and competence development.

Telephone advice is the entry point for our patients, and accessibility is incredibly important for the entire health center to function. We consider it important that all nurses maintain their competence in telephone advice and that everyone can contribute to this work when needed. Therefore, you will also work to some extent with telephone advice in an extent we agree upon.

About You

We are looking for you who are a licensed nurse/registered district nurse and have experience in diabetes care. Primary care experience and district nurse education are meritorious, as is education in the diabetes field.

You are a positive person with great flexibility and who finds it easy to collaborate. You are engaged and interested in primary care. You enjoy being part of a larger context and in your professional role, you are empathetic and value good relationships with other people. You are also a person with good structure and prioritization skills.
