Description
Do you want to make a concrete difference by strengthening people's path to self-sufficiency?
In 2026, activity requirements will be introduced within the social assistance system. This means that more people will need meaningful, individually tailored activities that strengthen the path towards work, studies, and self-sufficiency. The Employment Unit in Vänersborg plays a central role in implementing this practically and is now looking for dedicated and responsible employees who wish to contribute with structure, drive, and collaboration.
We are now looking for two Case Managers for the Employment Unit (AME) in Vänersborg Municipality. You will become part of an interdisciplinary team together with job coaches, occupational therapists, and health guides, where collaboration and joint planning are central to the work.
About the Employment Unit
The Employment Unit (AME) works to strengthen individuals' opportunities for self-sufficiency through work, studies, or labor market preparatory measures. We meet people with various types of barriers to income and work from a holistic perspective where the individual's needs, conditions, and goals are at the center.
The work is carried out in interdisciplinary teams where several competencies collaborate around the participant. As a Case Manager, you contribute with coordination and process leadership so that measures from job coaching, employment counselors, occupational therapy, etc., connect and become understandable, feasible, and follow-up capable.
Responsibilities
As a Case Manager, you are a key person in the participant's process. You work closely with the participant and together with the team based on needs, resources, and goals. Your task is to create context, structure, and direction on the path to work or studies, and to ensure that planned measures are implemented, followed up, and adjusted as needed.
The role includes:
- Coordinating the participant's planning and measures in close collaboration with other colleagues at AME.
- Conducting mapping, creating joint planning together with the participant, and following progress towards agreed goals.
- Strengthening motivation and participation through a coaching and solution-focused approach.
- Collaborating with internal and external actors, such as Social Assistance, the Swedish Public Employment Service, healthcare, and educational providers, to create cohesive measures.
- Documenting, following up, and analyzing measures, as well as initiating adjustments together with the team based on the participant's development.
You function as a cohesive link in the participant's process and contribute to structure, continuity, and progression. The assignment also includes keeping communication within and around the team together so that the participant receives consistent support with clear direction.
Qualifications
We are looking for you who have:
- A university degree in fields such as social work/social services, behavioral science, rehabilitation, or other education that the employer deems equivalent.
- Experience in coordinating individual-oriented measures (e.g., coaching, rehabilitation, or labor market measures).
- Experience working with people who are far from the labor market and/or have complex needs.
- Good familiarity with documentation, follow-up, and working structurally based on planning and goals.
- Good knowledge of authority collaboration and understanding of relevant regulations (e.g., social assistance, the Swedish Public Employment Service, health care).
- A valid driver's license (Category B) is required.
Personal Qualities
The work requires that you thrive in a role where you both work closely with the participant and coordinate measures in an interdisciplinary team. You need to be able to switch between operational support, structure in processes, and relationship-building collaboration.
We see that you:
- Have the ability to create trust and motivate change.
- Are structured, clear, and good at keeping planning and follow-up together.
- Are proactive, solution-focused, and confident in making decisions within your assignment.
- Have very good collaboration skills and can contribute to a common holistic perspective in the team.
To succeed in the assignment, you need to enjoy working independently while simultaneously being an active team player. You are goal-oriented, have good prioritization skills, and can create and maintain trusting relationships with participants and collaborating partners.
What We Offer
With us, you have the opportunity to work in an operation with an important societal mission and contribute to developing working methods that make a real difference for people. You become part of an interdisciplinary team at AME where we work closely together, learn from each other's competencies, and maintain a clear focus on collaboration and a holistic view of the participant.
