Your Mission

With us, you will play an important role in investigating, deciding, and following up on assistance and interventions according to the Social Services Act (SoL) and the Act on Support and Service for Certain Disabled Persons (LSS). Together, we develop our operations and actively contribute to Söderhamn's municipal vision and community development.

You will be part of a competent and supportive team where collaboration is a given. You have your own office and work closely with colleagues, managers, and specialists in regulatory activities – an environment that promotes both quality and job satisfaction. In our work, we use IBIC (Individual Needs in Focus), a structured and needs-oriented approach that ensures legally secure processing and clear documentation.

As a care manager with us, you will have a central and significant role where you:

  • Meet people in various life situations and build trusting relationships.
  • Investigate and assess the need for various forms of home care services, nursing homes, day activities, and other individually tailored support measures.
  • Make independent and legally secure decisions according to applicable legislation.
  • Follow up and evaluate granted measures to ensure that the right support is provided at the right time.

Workplace Description

The Care Unit is part of the Regulatory Department within the Welfare Sector, where we collaborate with the Financial Support Unit and the Adult Unit. Our unit consists of a dedicated team of care managers, LSS and care managers, housing coordinators, specialists, and unit managers.

Within the Welfare Sector, approximately 1100 employees meet people in various life situations through their professional roles in, for example, nursing homes, home care, home health care, LSS housing, social psychiatry, addiction centers, and financial support units.

Your Competence

We are looking for someone with a university degree in social work or another relevant education that the employer considers equivalent. You have good knowledge of regulatory activities and a solid understanding of applicable legislation.

Experience as a care manager or LSS case manager is an advantage. It is also beneficial if you have experience with the Lifecare operational system and knowledge of working according to IBIC (Individual Needs in Focus).

You have a good ability to express yourself in Swedish, both orally and in writing. Since travel is included in the position, a B driver's license is required.

As a person, you see opportunities and are willing to take initiative. You are eager to share your knowledge and experiences with colleagues and actively contribute to a positive work environment. Furthermore, you are a good analyst who is skilled at seeing connections and solving complex issues and problems. It is also important that you can plan, organize, and prioritize your work effectively, as well as set and meet deadlines.
