JOB RESPONSIBILITIES
New Safe Discharge is a service that encompasses Safe Discharge in both regular housing and short-term accommodation, characterized by a rehabilitative approach. The goal is for the customer to achieve full potential regarding independence, participation, and activity ability. Safe Discharge means that a team meets the customer directly after discharge from inpatient care or receives customers in the short-term department. The team provides support and assistance during the initial period after the hospital stay, usually for two to three weeks.
Within the service, all care staff work in the team towards both regular housing and short-term accommodation, and all work with customers is conducted with a rehabilitative approach.
You will work in a team with nurses, occupational therapists, physiotherapists, and case managers, focusing on preventive, rehabilitative, and person-centered work.
The customer should be able to receive support from New Safe Discharge for a limited time so that the transition to home care/return to regular housing is safe and secure.
Your assignment will be to promote the individual patient's own resources in daily care and activities, working with a rehabilitative approach.
Solo work may occur.
Some shifts may be scheduled within a nearby unit with similar tasks.
QUALIFICATIONS
We are looking for you who are a trained care assistant with a vocational degree from the health and care program or approved grades from adult education courses in health and care totaling 1500 points.
It is advantageous if you are a specialist care assistant in rehabilitation.
You are confident in your professional role and used to working closely with others. You work based on a rehabilitative approach and set goals.
Experience working in municipal services and rehabilitation, as well as good documentation skills in operational systems, is advantageous.
You should have a clear focus on applying a rehabilitative approach and how to promote the individual's own resources.
With commitment, a flexible attitude, and a pedagogical and communicative approach, you enjoy creating security in interactions with customers and colleagues.
You have a good demeanor and thrive in an environment where you can work independently while contributing to a strong team.
You are confident in your professional role and have good collaboration skills and take responsibility for your work.
Within New Safe Discharge, a good understanding and knowledge of the customer's participation in their own rehabilitation process is required.
You are empathetic and attentive and express yourself well in Swedish, both orally and in writing.
Great emphasis will be placed on personal suitability.
A B driver's license (manual) is required for the position.
Before employment, you must be able to prove your right to work in Sweden by presenting Swedish citizenship, EU/EES citizenship, or a valid residence and/or work permit.
For employment, a police record extract is also required.
