Your Role with Us

We are looking for nursing students who have completed semester 5 for a developing and rewarding summer job as an SVU Coordinator in the operational system Mina planer during the period of June to August. We offer an educational summer job where you will have the chance to deepen your understanding of collaboration between different levels of care and gain good insight into the care planning process. With us, you will be part of an engaged team and receive thorough introduction and continuous support throughout the assignment, so you can feel secure and grow in your role.

As an SVU Coordinator, you will work daytime, Monday to Friday, with coordinated care planning in the operational system Mina planer. In this role, you will serve as an important link between inpatient care, primary care, and municipal services. Through the system, you will communicate with various care providers and coordinate planning for citizens who need care and support in connection with and after a hospital stay. The work involves following the care planning process, ensuring that information is transferred between relevant parties, and contributing to timely planning so that the individual can have a safe and secure discharge.

Who Are We Looking For?

We are looking for you who are a nursing student and have completed semester 5 before the summer. You have good computer skills and feel comfortable working in digital systems. As a person, you are structured, responsible, and have good collaboration skills. You have a professional demeanor, good communication skills, and express yourself well in Swedish, both verbally and in writing. Experience in health care or municipal care is an advantage. It is also an advantage if you have a B driver's license.

The employment is a summer temporary position during June to August with daytime working hours, Monday to Friday.

This Is Us

The Care Administration is undergoing extensive innovative change work. We are looking for you who want to continue developing the Discharge Support Team in Kristianstad Municipality. The team consists of case officers, nurses, occupational therapists, physiotherapists, and nursing assistants. The team comprises 30 employees. You will also have close cooperation with nurses in short-term care and be able to support them as needed.

We base our work on patient-centered care, striving for teamwork together with physiotherapists, occupational therapists, nursing assistants, and case officers to meet the patient's needs. The organization also has access to specialist nurses in psychiatry and dementia. Our vision is a unit with broad competence where we provide the best possible care to our patients in a cross-professional manner.

The Care Administration's operations are aimed at the elderly and people who need functional support as well as those in need of home healthcare. The operations are functionally divided into four areas: support in ordinary housing, support in care and nursing homes, health care, and functional support. The administration also includes the authority unit, national gymnasium, and assistive technology center in Eastern Skåne.
