Your role with us

The Home Care Support Team is responsible for the patient process (collaboration during discharge), which means planning in connection with hospital stays, where the aim is for customers in need of continued care and support to be able to return home after hospitalization.

The team is also responsible for the patient process when the Home Care Support service is granted, from planning to execution over approximately 3 weeks. Recovery, training, and rehabilitation will take place in the home environment. The team has a close collaboration with Region Skåne.

You will work with medical assessments, medication management, support, and advice to the nursing staff. You will also spend a large part of your workday administratively in the shared IT system, My Plans, where all planning for collaboration during discharge takes place. You will also participate in coordinated individual planning (SIP).

As a nurse, you are tasked with coordinating and planning the combined health and medical care efforts for the individual patient and maintaining contact with relatives. We are looking for someone who enjoys working independently in collaboration with other healthcare staff. An important part of the job is to work in a team around the patient to achieve a good overall perspective. We offer an exciting and challenging job.

Who are we looking for?

We are looking for a licensed nurse. We prefer that you have work experience in the profession with experience in care planning teams and/or work in multidisciplinary teamwork. The ability to delegate and mentor nursing staff in a pedagogical and informative manner is an advantage.

Since communication and documentation are important parts of the job, we want you to be computer literate and proficient in Swedish, both spoken and written. Familiarity with IT-based journal systems is a requirement, as well as a B driver's license for manual transmission.

As a person, you have the ability to work both independently and together with others, to listen, show empathy, and respect. You are unpretentious and recognize and reward others' contributions. You have the ability to plan and organize your work and find it easy to set goals and follow up and complete activities.

The Social Care Administration is undergoing extensive innovative change work. We are looking for someone who wants to continue developing the Home Care Support Team in Kristianstad municipality. The team consists of assistance officers, nurses, occupational therapists, physiotherapists, and nursing assistants. The team consists of 30 employees.

You will also have close cooperation with nurses in short-term care and be able to support them as needed.

We base our work on patient-centered care, striving for teamwork together with physiotherapists, occupational therapists, nursing assistants, and assistance officers to meet the patient's needs. The organization also has access to specialist nurses in psychiatry and dementia. Our vision is a unit with broad competence where we provide the best possible care to our patients in an interdisciplinary manner.

The Social Care Administration's activities are aimed at the elderly and people who need functional support as well as those in need of home healthcare. The operations are functionally divided into four areas: support in ordinary housing, support in care and nursing homes, health and medical care, and functional support. The administration also includes the authority unit, the national gymnasium, and the assistive technology center in Eastern Skåne.
