Do you want to make a difference in people's everyday lives and contribute to a safe and professional coordination? We are now looking for an engaged case officer for our municipality.

The Assistance Unit is part of the Health and Care area and consists of dedicated and competent employees who work with case management according to SoL and LSS as well as fee management. We offer a workplace where collaboration, knowledge exchange, and a good working climate are central parts of everyday life.

As a case officer, you will be part of the homecoming team that works with individuals who are to be discharged from the hospital and need support to return home safely.

As one of our colleagues is going on parental leave, the position is a temporary contract with the possibility of extension.

Qualifications

To succeed in the role of case officer, we see that you have:

• A degree in social work or another academic education that the employer deems equivalent.
• Good knowledge of Swedish, both spoken and written.
• Good computer skills.
• A valid driver's license (B).

It is advantageous if you have previously:

• Experience in authority exercise within social services.
• Experience in case management according to LSS.
• Previous experience or knowledge of operational systems within social work.

Responsibilities

As a case officer, you are responsible for authority exercise according to the Social Services Act (SoL). Your assignment includes:

• Investigating, assessing, and making decisions about assistance measures.
• Following up and re-evaluating decided measures.
• Documenting according to applicable legislation and guidelines.
• Collaborating with individuals, relatives, and other professionals, as well as cooperating with internal and external actors and providers.
• Contributing to the development work and quality assurance of the organization.

The work involves independent responsibility while you are part of a team where we support each other and work together to create the best possible conditions for the municipality's residents.
