Care Coordinator, Askersund Health CenterJob Responsibilities

As a care coordinator, you work to ensure the individual's right to security, participation, and influence in the planning and implementation of care, support, and assistance. You are the patient's initial fixed care contact and guide them while creating security throughout the care process. If necessary, you initiate and coordinate contacts with relevant professional groups within the region and municipality. You have a central role in the organization's mission to work in accordance with the law on safe and secure admission and discharge from inpatient care.

In this role, you are the coordinating link between inpatient care, primary care, and municipal services. One of your main tasks is to work according to the ViSam model. You are responsible for calling and leading coordinated individual planning (SIP), where you together with the patient and any relatives develop a common plan for a safe and secure everyday life.

You also have a driving role in the transition to Close Care, where the focus shifts from organization to person and relationship. Together with colleagues, you actively work to develop collaboration with the municipality, creating conditions for a more person-centered and resource-efficient approach around the patient. The role also includes reception work at the health center based on your core profession.

Qualifications

You have education and experience in healthcare, or other education that the employer deems relevant for the position. Experience in similar coordinating roles is advantageous, as is experience working according to the ViSam model. It is also advantageous if you have experience with information transfer systems between healthcare providers, such as Lifecare. You have good digital skills and feel comfortable using various digital tools in your work.

As a person, you are independent, structured, and confident in your professional role. You have good communication skills and enjoy collaborating with others. You have a genuine interest in patient-centered work and want to contribute to development - both of the organization, the workgroup, and your own professional competence. You find it easy to create trusting and positive meetings with both patients and colleagues. We place great importance on personal suitability.
