During rush hour, it takes about 30 minutes by direct bus from Slussen. You start your day with a short journey over bridges and sea to our beautiful archipelago municipality. Buses run every 10 minutes. In our central locality Gustavsberg, you will find our bright and fresh municipal building. If you choose to drive instead, the same distance takes 20 minutes.

At the Social and Care Office, Värmdö municipality, two Case Managers work alongside five social workers, a group leader, and a unit manager.

One of our Case Managers will go on parental leave from April 2026 to April 2027, and we need a temporary reinforcement.

In the role of Case Manager, you will have your workplace at Värmdö municipal building. You will work on behalf of social workers in your team and have close cooperation with the municipality's Addiction Reception. In the role of Case Manager, you have a collaborative role with psychiatry and the county's addiction care.

Your target group is adults who have addiction issues and comorbidity. The foundation of your mission is to be coordinating and to contribute to ensuring that the individual receives adequate support and a more independent life. The position is also linked to the unit's training apartments.

We are looking for someone who is a social worker or has another equivalent academic education that the employer deems relevant for the assignment. It is an advantage if you have previously worked as a Case Manager.
