We are looking for a dedicated and qualified case officer for individual and family care (IFO) in Laxå municipality.

Responsibilities

In the role of case officer, you will be responsible for investigating applications, making decisions, and following up on granted services within elderly care, LSS, and social psychiatry. You will make decisions according to the Act on Support and Service for Certain Disabled Persons (LSS) and the Social Services Act (SoL). You will have a central coordinating role during discharge from the hospital and ensure that services are planned and followed up in collaboration with relevant actors. Our mission is to meet the needs of our users for services to contribute to an independent life based on the individual's circumstances. We use the operational systems Treserva and Lifecare in our daily work. You will have many contacts with various partners both internally and externally.

Qualifications

We are looking for someone with a degree in social work or another equivalent education and experience working as a case officer in elderly care and/or LSS. We prefer that you have a B driver's license. It is important that you have good communication skills and the ability to establish trusting relationships with users and their relatives.
