Who are we?

The Discharge Team in Norrköping is based at Hantverkaregatan 51 and consists of nurses, occupational therapists, physiotherapists, and care assistants.

The team's mission is to provide support with care and assistance, including rehabilitation, to individuals with extensive and changing needs that cannot currently be met by regular home care. The need for the discharge team's services may arise after a hospital stay, a stay at a short-term accommodation, or in connection with home care matters in the individual's own residence.

Your tasks

Your work as a care assistant in the Discharge Team is important and you influence how others experience their daily lives. You provide security and support with your expertise upon discharge after a hospital stay.

You will be part of a team where the individual’s need for care and support is prioritized. Together, the team creates a sense of security around the individual with a focus on a rehabilitative approach.

As a care assistant in the Discharge Team, you will work days, evenings, and weekends.

Who are you?

We are looking for someone who is a trained care assistant, according to the regulations of the National Board of Health and Welfare. You have experience with standard care assistant tasks. We value a positive attitude, commitment, flexibility, and a sense of service. As a person, you are secure, stable, and responsible.

You have the ability to adapt your approach to the situation and you structure and drive your work both independently and in collaboration with the team.

You are attentive and able to understand other people's situations. You adapt easily to changing circumstances and see changes as an opportunity.

You are proficient in the Swedish language, both spoken and written. You have a driver's license and can ride a bike.
