Stora Sköndal's Neurological Rehabilitation Clinic offers active and intensive rehabilitation for patients with stroke, Parkinson's, MS, and other neurological diseases and injuries. Rehabilitation is a team effort where speech therapists, occupational therapists, physiotherapists, social workers, psychologists, doctors, nurses, and nursing assistants work together with the patient and their relatives. In addition to inpatient rehabilitation, we also receive patients in outpatient care at our neurological day rehabilitation and at our speech therapy and physiotherapy clinic.
We are now looking for a substitute for our social work group as one of our regular social workers will be on study leave. You will primarily work in our 24-hour rehabilitation with adult patients who come to us after care in acute hospitals. Patients often find themselves in a new life situation, for example after a stroke. Your task is to assess the need for social support that has arisen and guide the patient to the right municipal services as well as offer supportive conversations. Together with the rest of the team, you are responsible for ensuring that the patient has a safe and secure transition home after their rehabilitation period with us. You will also have part of your duties in our day rehabilitation where patients come for intensive training for 2-3 months. The social work interventions there can have a more conversation-oriented character, but here too, a large part of the task is to guide to the right services and support from society.
The substitute position is full-time and lasts until the end of the year, starting as soon as possible. There are good opportunities for an extended substitute position at the turn of the year. Applications and interviews will be handled on an ongoing basis, and the position may be filled before the final application date of 2026-02-16.
You are a trained social worker, preferably with experience working as a social worker in healthcare. Good knowledge of the Swedish language and experience in documentation are required. The work places high demands on organizational skills and the ability to collaborate. Great emphasis is placed on personal suitability.
