We are currently seeking a nurse for our resource team within the Care and Nursing Home!

The municipal healthcare is in an exciting phase with new opportunities linked to the needs of our citizens. Not least, the major Close Care transformation is underway, where more of the qualified care is managed by us in the municipality.

As a nurse in the resource team, you will work both day and evening based on the needs in our 39 special accommodations spread throughout Malmö - from Tygelsjö in the south to Värnhem in the north.

Job Responsibilities

As a resource nurse, you are responsible for the care of patients. This means that you independently assess, plan, and organize medical and nursing interventions. You are responsible for mentoring nursing staff. You work person-centered by involving the patient in decisions and collaborating closely with relatives and the team around the patient.

You regularly collaborate with primary care through rounds based on a patient focus. You will also collaborate with several actors according to the health and medical care agreement, where the county council, municipality, and primary care are included to meet the individual’s care needs.

The Care and Nursing Home is divided into five sections with several accommodations per section where we systematically work with quality assurance systems BPSD, SveDem, Senior Alert, and Palliative Register. Our goal is to provide as personal and seamless health and medical care as possible with high quality and presence. Our Care and Nursing Homes focus on somatic and/or dementia care.

Qualifications

We are looking for you who are a licensed nurse, with at least two years of work experience. In addition, you have:

  • Very good knowledge of the Swedish language, both spoken and written
  • Insight into applicable legislation, establishing goals and guidelines
  • Good digital competence
  • Driver's license B

It is meritorious if you have:

  • Work experience with individuals who are multi-sick and/or have dementia
  • Work experience from municipal health and medical care
  • Specialist education in a relevant field
  • Good experience in collaborating with other authorities and being part of various networks
  • Knowledge of the documentation program Procapita
