We are looking for someone who wants to work with coordinated care and support planning in connection with hospital discharge.
In this role, you will be responsible for receiving and managing messages in SAMSA, participating in digital care planning, and ensuring that the right support is in place when the individual returns home. You will assess needs and decide on interventions according to SoL, collaborate closely with hospitals, primary care, and the municipality's providers, and document the planning in SAMSA. The work is varied, requires good collaboration skills, and means that you are a key person for a safe and secure discharge for our residents.

At the Adult Unit, we work with authority exercises in several areas. The unit currently consists of 14 employees and is led by a senior social worker and a unit manager.

Job responsibilities
- Collaborate closely with relatives, caregivers, health and medical care, other authorities, and other partners.
- Assess needs, make decisions, and follow up on interventions according to SoL.
- Focus on care and support, but other areas may also arise as needed.
- Work from a holistic perspective where the individual's needs and participation are central.
- The role also includes being present at the health center certain days of the week to provide early support through guidance, advice, and information to our residents.
In meetings with users and relatives, a safe, professional, and empathetic approach is crucial.
Competencies and abilities
To succeed in this role, you need to have:
- A good ability to work independently and in a structured manner.
- Flexibility and confidence in handling varied cases.
- A clear and trust-building communication style, both verbally and in writing.
- The ability to understand other people's situations without taking over their feelings.
- Initiative and responsibility, both in collaboration and in independent work.
- A good understanding of the importance of collaboration, both internally and externally.
Qualifications
- A degree in social work or other education that the employer assesses as equivalent.
- Experience in authority exercises according to SoL within care and support and SAMSA.
- Experience with IBIC.
- A valid driver's license (B).
