We are now looking for licensed nurses who want to work as hourly substitutes in the Mobile Team Primary Care. Our operation is part of Region Skåne's transition to closer care and targets patients who need qualified healthcare at home instead of in the hospital.
The Mobile Team Primary Care acts as a coordinating hub, assessing, planning, prescribing, and providing care interventions in the patient's home environment. Care is provided based on each individual patient's needs, with the goal of offering both medical and psychosocial care of high quality. The team consists of various professionals working together, including doctors, nurses, social workers, and medical secretaries.

RESPONSIBILITIES
As a nurse in the Mobile Team Primary Care, you will work both independently and in close collaboration with other professionals in the team. The work involves planned home visits as well as joint team visits, where you meet patients in their regular or municipal housing.
Responsibilities may include sampling, medication administration including intravenous treatment, nursing interventions, as well as conversations and support for patients and relatives. You play an important role in planning and coordinating care interventions together with other healthcare providers.
The work is conducted during the day, and the operation is open every day of the week, including weekends.
The position is for hourly employment in various locations; please indicate in the survey which locations you are interested in.
QUALIFICATIONS
We welcome you who are a licensed nurse with the authority to practice within Swedish healthcare. You have good knowledge of the Swedish language, corresponding to at least level C1 according to the Council of Europe’s scale. Since the work involves home visits, a valid B driver's license is required. It is an advantage if you have several years of experience in somatic care.

This includes mobile teams and specialized palliative care, outpatient activities within close care, acute care within close care, 1177 by phone, and certain region-wide functions. Additionally, the department is responsible for the development and follow-up of the transition to close care in Region Skåne. The number of employees is estimated to be around 1,000.
