We are currently looking for specialist doctors who want to work as substitute staff in the Mobile Team for Primary Care. This initiative is part of Region Skåne's transition towards closer healthcare and is aimed at patients who need qualified medical care at home instead of in hospitals.
The Mobile Team for Primary Care acts as a coordinating hub, assessing, planning, prescribing, and delivering care interventions in the patient's home environment. Care is provided based on each individual patient's needs, with the goal of offering both medical and psychosocial care of high quality. The team consists of various professionals working together, including doctors, nurses, social workers, and medical secretaries.

RESPONSIBILITIES
As a specialist doctor in the Mobile Team for Primary Care, you will conduct home visits and joint team visits, where the patient receives medical assessment and treatment at home that would otherwise have been performed in a hospital. You will lead and participate in the planning and follow-up of care interventions together with the team and collaborate with other healthcare providers and partners to ensure safe and cohesive care.
The work involves medical assessments, prescriptions, and follow-up of patients with complex care needs. You will also contribute to the development and improvement work of the organization.

The work is conducted during daytime hours, and the organization is open every day of the week, including weekends.

QUALIFICATIONS
We are looking for a licensed physician with the authority to practice within Swedish healthcare and who has specialist competence in general medicine, geriatrics, internal medicine, or another equivalent specialty. You have good knowledge of the Swedish language, corresponding to at least level C1 according to the Council of Europe's scale. A valid B driver's license is a requirement as care is primarily provided in patients' homes.
Experience in similar work and knowledge of the medical record systems PMO, Melior, and Pascal are advantageous.
As a person, you are confident in your professional role, have a good ability to assess care levels in sometimes seriously ill patients, and always put the patient first. You enjoy working in a team, are communicative, and have the ability to meet patients and relatives in various life situations. You have a flexible approach and can adapt to the needs of the organization. Great emphasis is placed on personal suitability.

This includes mobile teams and specialized palliative care, outpatient services within close care, emergency care within close care, 1177 by phone, and certain region-wide functions. Additionally, the department is responsible for the development and follow-up of the transition to close care in Region Skåne. The number of employees is expected to be around 1,000.
