The operation is part of Region Skåne's transition to closer care and targets patients who need qualified medical care at home instead of in the hospital.

The Mobile Team for Primary Care acts as a coordinating hub and assesses, plans, prescribes, and performs care interventions in the patient's home environment. Care is provided based on each individual patient's needs with the goal of offering both medical and psychosocial care of high quality. The team consists of various professionals working together, including doctors, nurses, social workers, and medical secretaries.

JOB RESPONSIBILITIES

As a specialist doctor in the Mobile Team for Primary Care, you will work with home visits and joint team visits, where the patient receives medical assessment and treatment at home that would otherwise have been performed in the hospital. You will lead and participate in the planning and follow-up of care interventions together with the team and collaborate with other healthcare providers and partners to ensure safe and cohesive care.

The work includes medical assessments, prescriptions, and follow-up of patients with complex care needs. You will also contribute to the development and improvement work of the organization.

The work is conducted during daytime hours, and the operation is open every day of the week, including weekends.

QUALIFICATIONS

We are looking for you who are a licensed physician with the authority to practice within Swedish healthcare and who has specialist competence in general medicine, geriatrics, internal medicine, or another equivalent specialty. You have good knowledge of the Swedish language, corresponding to at least level C1 according to the Council of Europe's level scale. A valid B driver's license is a requirement as care is mainly provided in patients' homes.

It is advantageous if you have experience of similar work and knowledge of the medical record systems PMO, Melior, and Pascal.

As a person, you are secure in your professional role, have a good ability to assess the level of care for sometimes seriously ill patients, and always put the patient in focus. You enjoy working in a team, are communicative, and have the ability to meet patients and relatives in various life situations. You have a flexible approach and can adapt to the needs of the organization. Great emphasis is placed on personal suitability.
